We provide both health and financial resources to ensure safe, stable, and meaningful community living for our clients and their families.

Family Member Support

We help families manage the challenges of caring for someone with a disability or chronic illness. Our care managers and nurses provide consistent oversight and support. We're here for you - 24/7 - 365 days a year.

Who Qualifies?

Our clients are adults (16+) who have chronic illnesses or disabilities, cannot safely live alone, and need help with daily personal care.

Clients Must:

  • Live with a qualified caregiver (not a spouse or legal guardian)

  • Be eligible for MassHealth

  • Need help with at least one Activity of Daily Living (ADL): walking, eating, toileting, dressing, bathing, or transferring

Up to $1,700/month depending on the level of care required.

What is involved in screening?
We follow Massachusetts regulations. Screenings include interviews, medical clearance, reference checks, criminal record checks, and home inspections.
