Family Member Support
We help families manage the challenges of caring for someone with a disability or chronic illness. Our care managers and nurses provide consistent oversight and support. We're here for you - 24/7 - 365 days a year.
Who Qualifies?
Our clients are adults (16+) who have chronic illnesses or disabilities, cannot safely live alone, and need help with daily personal care.
Clients Must:

Live with a qualified caregiver (not a spouse or legal guardian)

Be eligible for MassHealth

Need help with at least one Activity of Daily Living (ADL): walking, eating, toileting, dressing, bathing, or transferring

We follow Massachusetts regulations. Screenings include interviews, medical clearance, reference checks, criminal record checks, and home inspections.
